With the impending launch of tablet versions of popular weight-loss medications, currently only available as injections in the UK, a potential flood of counterfeit pills is on the horizon. The demand for weight-loss jabs like Wegovy and Mounjaro has been steadily growing in the UK, with impressive results shown in clinical trials. But here's where it gets controversial: the ease of manufacturing these pills also means they are more susceptible to counterfeiting. Dr. Bernard Naughton, an associate professor at Trinity College Dublin, warns that counterfeit pills could contain medications from illegitimate sources, stored improperly, or even be contaminated or incorrectly dosed. "We've already seen examples of falsified injections," Naughton says. "Creating a fake pill is relatively simple, and with good packaging, it's easy to pass it off as the real deal."
And this is the part most people miss: patients are more likely to take risks online when it comes to tablets than injections. Naughton's research suggests that people are willing to overlook potential red flags and purchase tablets online, even if they suspect something might be amiss. This behavior highlights the need for better education and awareness among consumers.
Novo Nordisk, the producer of Wegovy, emphasizes that their pill contains not only the active ingredient semaglutide but also SNAC (salcaprozate sodium), which enhances drug absorption. "Only Novo Nordisk manufactures FDA-approved semaglutide medicines with this SNAC technology," a spokesperson clarifies. "Compounded semaglutide may contain impurities and untested doses."
The UK's Medicines and Healthcare products Regulatory Agency (MHRA) has previously issued warnings about the importance of purchasing weight-loss jabs from registered pharmacies or legitimate retailers. This precedent highlights the ongoing battle against fake weight-loss medications.
Oksana Pyzik, an associate professor at the UCL School of Pharmacy, notes that the demand for medications like Wegovy and Mounjaro has outpaced regulated supply, leading some to seek cheaper alternatives online through unlicensed routes. "Oral weight-loss drugs may improve access, but they also widen the opportunity for criminals to falsify GLP-1s," she warns.
In October 2025, the MHRA conducted a raid on a factory in Northampton, seizing counterfeit weight-loss jabs worth over £250,000. This incident underscores the growing sophistication of domestic counterfeit operations. Patel believes that while the MHRA is proactive, the widespread use of social media and online marketplaces poses a significant challenge in controlling the distribution of these counterfeit products.
Eli Lilly and Novo Nordisk are taking action to combat counterfeits, employing strategies such as online monitoring, sharing information with authorities, and providing sample pills for testing. Andy Morling, deputy director of enforcement at the MHRA, emphasizes the rigorous standards that authorized medicines must meet before use in the UK. "We are committed to ensuring the safety and effectiveness of all treatment formats," he says.
Pyzik, who is also the academic chair of the Fight the Fakes Alliance, advocates for stronger regulation and enforcement, including the use of AI tools to detect falsified online adverts. "Patients should only access these products with a prescription and under medical supervision," she advises. "Awareness campaigns targeting different risk attitudes are crucial."
Naughton suggests that the UK should make it easier for patients to verify the legitimacy of online pharmacies and develop targeted public health campaigns. "Buying medicines online has its benefits, but we must ensure it's done safely and responsibly," he concludes.
